Complete the appropriate claim form:

• Automobile Insurance

• Homeowners Insurance

• Business Owners Insurance

When completed, click on the Submit button to send the information to Shuffain Insurance Agency via email.
 

For automobile claims, click here to access the official Massachusetts Registry forms.  Complete the Operator's Report of Motor Vehicle Accident and mail it to the Registry of Motor Vehicles, the local police where the accident occurred and to Shuffain Insurance Agency.
